How to file a claim
Step 1. Notify us of the death
A good first step is to contact your loved one’s Modern Woodmen financial representative … or your own if that’s a different person. He or she can guide you through the claim process, help you fill out necessary forms and offer assistance where needed.
If you need contact information, go to the Find a Representative section of this website and enter the representative’s name. Or enter your loved one's ZIP code or city/state to access a list of representatives.
You can also start the claim process by:
- Calling our home office Service Center at 309-558-3077 or 800-447-9811, Monday through Friday, 8 a.m. to 4:30 p.m. (Central).
- Completing this online form.
Step 2. Submit the claim forms
Once you (or your representative) have notified Modern Woodmen’s home office of the death, we will verify if any benefits are owed. If so, then we’ll provide forms for the named beneficiary(ies) to complete.
Please send the completed claim forms and certified death certificate back to the home office.

Step 3. Receive payment
Modern Woodmen will promptly review the claim once we receive all necessary forms and documentation. Then we will make the appropriate payment, if any, to the beneficiary(ies).
