My Life, My Plan workbook
This resource helps individuals get organized and share their final wishes with their loved ones. The easy-to-follow guide breaks end-of-life planning down into manageable, bite-size chunks. Users work through the steps to help get their houses in order, organize their affairs and record their wishes. The workbook includes:
- Possessions inventory.
- Final-arrangements planner.
- Obituary template.
- Online accounts document.
- Life insurance needs analysis.

My Life, My Plan – A Helper’s Guide
This resource provides advice for those who want to help a parent, spouse, relative or friend prepare his/her final wishes. The guide offers tips for holding end-of-life planning conversations – from the helper’s perspective. It also helps prepare members for the eventual loss of a loved one, sharing:
- Steps to take after a death.
- Tips for handling a loved one’s affairs.
- Information about the grief process.
- Links to helpful resources.
Available as a printed booklet or fillable PDF.
Do I qualify?
To be eligible for either resource, you must be a member with Modern Woodmen life insurance or annuity coverage. (The certificate must be in your own name and in good standing.)
How to order
Contact your Modern Woodmen representative to request one or both resources. (You can find his or her contact information using the Find a Rep feature of this site.) Or contact the Fraternal Department at Modern Woodmen’s headquarters.
When you request the program, please indicate whether you want the hard copy (printed booklet) or digital (fillable PDF) version of the My Life, My Plan workbook, helper’s guide or both.
Please allow time for processing and delivery.
