Fraternal Leader Handbook and Code of Conduct​

Please review and acknowledge the Fraternal Leader Handbook and Code of Conduct. As the handbook is updated annually, it is mandatory for all fraternal leaders, including activities coordinators and youth club leaders, to review the handbook, sign the acknowledgment form, and complete this section each year. Follow the instructions provided to upload the signed acknowledgment form.​​

  2. Print and ​sign the acknowledgment form.
  3. ​​Scan and save signed document to your computer.
  4. Click "choose file" to upload your signed acknowledgment form.

I understand Chapter/Youth Club funds can only be used for the purposes set forth in the Handbook and cannot be used for personal purposes unrelated to the Chapter/Youth Club. I further understand and agree the Chapter/Youth Club and/or Modern Woodmen of America will take any necessary action to recover funds used for non-approved purposes.
